For your first question, I would recommend VerbalCare, but of course I am very biased (being the co-founder) :)  There are some other companies (Starling Health, Patient Provider Communications, etc), but I am unsure if they have worked with ALS patients in the past (our trial at MGH was with the ALS population).  Happy to talk with you if you're interested!  I would love to see how we could help!

In regards to your other questions, I have some suggestions, but I admittedly have used few ECUs to truly be an expert.  For Bluetooth, I know a lot of people who are happy with their RJ Cooper switches.
