These all seem like possible suggestions, but I just want to gently point out that this patient has severe ALS and in the ICU, so what he was able to accomplish at home he might not even be able to do in the hospital.

Personally, I would go with the "lowest" level of demand, which is probably letter board scanning by row or group, and then narrow to specific letter.

Here is a video of someone using a laser with a large core vocabulary board... She has vey limited head movement, but because of the distance of the board she is able to easily use it.
