USB. It is programmable and also is capable of being programmed with keyboard shortcuts, that can then be used to activate macros, so it offers a lot more flexibility (if a little more trouble than other to maintain).

If you really just want to switch activate the mouse click I often recommend against a USB switch activated mouse as they are always tending to move on me. I prefer a direct switch interface such as the hitch from Ablenet http://www.ablenetinc.com/Assistive-Technology/Computer-Access/Hitch .
