I know there is a bluetooth headset for Dragon, but it cannot do double duty as a cell phone headset. 

There are some ways to switch between dictation and telephone (land line) use, and there may be an option for cell phone, but all of these solutions would require physically pressing a button.

But, there is an app that allows you to use your cell phone as the microphone for Dragon - http://www.nuance.com/dragon/whats-new-upgrade/index.htm (click on Dragon Remote Microphone)

Another solution would be to use a desktop mic that does not require don/doff - this one is very good quality - http://www.emicrophones.com/p-1112-buddy-gooseneck-usb-microphone.aspx  - It has a long gooseneck.  This is what I typically recommend for those that cannot put on/off a headset.  Be aware that the desktop mic should be used in a relatively quiet area as it will pick up background noise.

If you go with the desktop mic, then the patient can use a separate device for cell phone access.

FYI - when change mics, you will need to create a new profile for each sound source.

I don't use Dragon very much. (hardly at all for my ALS folks) But I do have an OT with ALS that comes to our clinic with good speech but bad hand/arm support.  He asks the following question and I am hoping someone knows the answer. Thanks in advance for the help!
 
I am currently using Dragon at work and at home to interface with word documents , work documentation and e-mail. As things have progressed with me it is almost impossible to put on a headset therefore I have started to use a microphone. The microphone however is getting harder to hold. In addition it's getting harder for me to hold my cell phone to my ear. Is there a way to solve both problems with some sort of a Bluetooth setup with wireless microphone set up to allow me to interface with Dragon and answer and use my cell phone?
