I had researched this in the past. From what I remember, this used to be possible. With the newer phones they have noise cancelling mics that make this not work. As far as I know this is not possible on current phones.

I think iPhone 4 may have been the last version that this would work for, but I am not sure. You can try it with Speak it! and see if the receiver can hear the text across the phone line.

Does anyone know of a TTS app that can speak during a phone call made on the same iPhone?

I am interested to see if this works on any model but specifically for an iPhone 4 since I currently have a client with that model who needs this ability. I have a feeling newer models allow for more simultaneous app usage but I hope her older model is not a problem.
