I am a big fan of Predictable ($160) and Assistive Express ($25) for iPad. Especially like that Predictable allows for some built in message banking.

Know less about Android options, but have one client who really likes "Speech Assistant" on her Android phone. Not many bells and whistles, and I'm not sure exact price, but I think it's pretty cheap.
