There are a couple of microphones on the market that require the user to switch between computer and phone by pressing a button on the base unit. Still need to use your hand, but do not need to reach to your ear. You could take a look at these:

Subject: Completely Hands-free Multipoint Headset

I'm working with a person with ALS who would like to find a completely hands-free headset to use with his cell phone and computer simultaneously: He needs to be able to switch between functions (his phone and Dragon Naturally Speaking on his computer) without having to bring his hand up to the headset.

Any ideas?
