You will need to get a doctor's prescription for the SGD Evaluation and then call BC/BS and get a prior authorization for the service. I do believe there may be an exclusion for insurance purchasing an SGD for a patient in a nursing home but you may have to check that with Lew Golinker. I have a question about insurance.  I have a patient that has BC/BS and is in a nursing home.  He is also pending Medicaid.  BC/BS is not paying anything towards the nursing home Â? would BC/BS pay for an AAC (assuming it is a covered item)?
