My pALS in this situation use their iPads w/a speaker phone and one of the speaking apps (Speak It, NeoKate, NeoPaul, AssistiveChat).  For louder voice, I suggest they buy an external speaker such as the Omnitech speaker from Staples (cost is $9.99).
