We use the FreedomAlert as it does not have a monthly fee.  We switch adapt the Emergency Wall Communicator ourselves for those who need switch access.  If we have someone that we want to use this with that does not have a land line, we recommend this item:

(you do have to be in bluetooth range of the device, fyi, so not best solution for everyone)
