I am no expert on these things.  But, when the iPhone first came out, you could do this.

I went down this rabbit hole a number of years ago, and the info I ended up with was that when the phones started adding noise cancelling to their mics, this prevented being able to hear the audio of the app over the phone line.  So, I think that it would require a deep mod on the phone to allow this - but I could be wrong.

Just yesterday, I called a client and when the call connected, my music started playing - a glitch.  I had to hang up on her can call her back.  But, we both heard the audio coming from iTunes and we could hear each other talking.  So there is hope...

Does California have a relay service, TTY (text telephone)? This is a service known more for the Deaf or Hard of Hearing. You wouldn’t be using a cell phone, but at least with this service the individual would use a 3rd person to relay typed message to caller.  In Iowa, it is called Relay Iowa.  People with other communication difficulties can also use. It is a free service to use and low to minimal cost for any equipment needed.  Just a thought.
